How to Review Your Medical Record on Your Time

This personal health record puts your healthcare information at your fingertips.

When you left the doctor’s office, you knew exactly how you were supposed to treat that nagging ailment. But in between picking up the prescription, dropping the kids off at their after-school activities, getting dinner on the table and helping with homework, you’ve forgotten those critical instructions.

Fortunately, a quick look at your free Health Check Connect account gives you the answers you need when you review your doctor’s notes from the visit.

Connecting patients with the same medical information that their healthcare providers see is the thinking behind Health Check Connect, a free personal health record offered by Delaware Health Information Network (DHIN).

As Delaware’s statewide health information exchange, DHIN provides a secure Community Health Record through which physicians’ offices, hospitals, labs and imaging facilities share clinical information, safely and effectively. Every acute care hospital in the state and nearly all of Delaware’s physicians who order tests participate, ensuring the timely delivery of test results and records to speed medical care while saving money and time.

The personal health information reported into DHIN’s Community Health Record—test results, doctors’ notes and imaging studies, for example—is now available through a secure portal for patients to see and to add to with prescription information, vital statistics and data from personal health trackers.

In the coming weeks, patients will be able to see their immunizations and receive text alerts when new information, like lab results, has been delivered, or when a member of their healthcare team has accessed their medical record.

“The interactive nature of Health Check Connect is key,” says Stacey Haddock Schiller, DHIN’s marketing director. “Patients can chart their progress towards reaching health goals, securely email healthcare providers and review a customized health dashboard.”
