Could Chiropractic Care Help You?

Patients and their primary-care physicians and specialists are increasingly looking to chiropractic care as a first-line option.

Patients are now looking more frequently to chiropractic care to treat headaches and pain in the back, neck and joints of the arms and legs.

 

The time of “rack ’em and crack ’em”—with sometimes less than desirable outcomes for patients—has passed in chiropractic care. 

“Back in the day, they might go to a chiropractor and end up leaving sore,” says Dr. Tracy Hudson, owner of Henlopen Chiropractic in Lewes. “That barely happens anymore. It’s not as aggressive as it used to be.”

Doctors of chiropractic (commonly known as chiropractors) today frequently use gentler means to restore the mobility of the joints ravaged from anything from birth trauma, to a car accident, to poor posture and daily stress. In addition, they have taken an increasingly holistic view of the body and pain. 

As a result, patients and, importantly, their primary care physicians and specialists now look more frequently to chiropractic care as a first-line option to treat headaches and pain in the back, neck and joints of the arms and legs, says Hudson, a chiropractor for 14 years. Research also shows chiropractic care can benefit a variety of other conditions (see sidebar).

Dr. Robert Hartmann admitted his skepticism of chiropractic care at first for his lower back pain. But after some rough days, the physician at Dedicated to Women ObGyn in Dover decided to give it a try rather than continue down the pharmaceutical route. Pleased with the results, he now frequently refers his prenatal patients who suffer from lower back issues to a chiropractor. 

“We’re limited in what medications we can prescribe because of the pregnancy,” Hartmann says. “Chiropractic care is an excellent alternative.” 

He also recommends the therapy for his general patients who may be battling lower back issues like sciatica. “It’s another effective modality for treatment,” Hartmann says.

40-year-old with back discomfort. 

Variety of patients

Like Hartmann, Dr. Becky Leary saw a chiropractor for back pain. Years of playing the bass drum at Dover High School led to these visits and, perhaps surprisingly, even a complete career switch. 

She originally planned to be a pharmacist, but after a few adjustments and quick resolution of pain, she decided to become a chiropractor instead. Leary attended Life University in suburban Atlanta for four years and received her doctor of chiropractic degree.

“It made sense,” she says of her job path, which landed her at Dover Family Chiropractic in Camden where she first went as a patient years ago. “You’re never going to be bored. Plus, you get to interact with all types of people.” 

In a given month, she may see patients that range from a 6-week-old infant with nasty colic, to a 40-year-old with back discomfort caused by his 9-to-5 desk job, to a senior citizen fighting through chronic neck pain. 

A complete evaluation

Leary’s patients will go through a detailed consultation on the first day, starting with a discussion of their overall health, such as lifestyle habits, stress level, diet and family medical history. She also will review any previous imaging done with a neurologist or orthopedist. 

“We just don’t ask you where it hurts, lay you down and ‘crack you,’” Leary says.

After the health history, Leary will perform an extensive whole-body physical exam, which includes checks of reflexes, paraspinal thermal imaging to assess nervous system function, a surface EMG to look at overall muscle function and a heart rate variability study (an indicator of how well the body deals with daily stress). 

“Pain is only a small percentage of the nervous system,” she says. “A lot of people hate their jobs or aren’t in healthy relationships. That will affect your overall health, too.”

And, of course, Leary does some initial “poking around” to pinpoint the areas of misalignment. 

“We take pride in our initial process,” she says. “You really don’t know what you’re working with until you take a closer look. We don’t even adjust them on the first visit.”

Dr. Gregory Yacucci finds immense value in the consultation exam.

Not for everyone

Dr. Gregory Yacucci, owner of Colonial Wellness and Rehabilitation Center in New Castle, also finds immense value in the consultation exam. He frequently works with patients with whiplash, sprains and strains, and prior injuries or surgeries that never completely healed, but sometimes, this discovery process reveals that people have come to the wrong office.

“I can’t help everybody,” says Yacucci, who has practiced since 1995. “That’s part of my job too—getting you to the right kind of doctor that can best treat you.”

For instance, seniors will sometimes visit with complaints of back pain; however, upon inspection, he sees they have shingles, a disease outside a chiropractor’s scope of practice.

Patients with severe arthritis may be beyond a chiropractor’s care as well, Leary adds. “With that level of degenerative changes, sometimes it’s tough to do anything to help.” 

She also sometimes struggles with patients dealing with other conflicts, be it a poor diet or inactive lifestyle.

“You can adjust them until the cows come home, but you have to get them on board with the other stuff going in their lives to support what you’re doing in the office,” she says. “It can be frustrating. You can only lead a horse to water.”

Quick and painless

Once the chiropractor and patient decide on a game plan for treatment, the adjustments will begin, Leary says. The majority of insurance companies will cover the sessions, but they may put a cap on the duration of care.

Most patients should start to see improvements after three or four 15-to-25-minute sessions, Yacucci says. Some tougher cases, including seniors and people with chronic health conditions, may take double that time for noticeable changes in pain and quality of life.

Along the theme of gentle mobilizing vs. “a big crack,” many chiropractors use a Food and Drug Administration-approved handheld instrument called the Activator to aid in the adjustment, Leary says. It produces an electronic thrust of pressure with no discomfort.

A drop table also becomes a valuable tool for chiropractors, Hudson says. Like the name implies, a section of the table drops out from underneath the patient as the chiropractor pushes down.

“That increases the speed of the adjustment, so we don’t have to be as forceful,” she says. “When you adjust something, you have to be fast, not necessarily hard.”

Despite all the advancements made in the field, Leary—a chiropractor for five years—admits she still faces concerns from worried patients, many who have lived in pain for decades.

“People say, ‘I was terrified to come here because I thought you were going to twist my neck off or crack my back,’” she says. “Once I adjust them, they’re like, ‘OK, that was silly.’”

WHAT DOES THE RESEARCH SAY?

An impressive—and growing—body of research details how chiropractic care goes beyond just helping neck and back pain or headaches. Here’s a small sampling of studies along with some anecdotes:

  • Autism: After 15 visits to the chiropractor, a 4-year-old boy with autism showed significant language development, increased sociability and cognitive improvements, a case study described. A larger paper, which reviewed five articles, saw the potential and encouraged further research on autism and chiropractic care. 
  • Colic: Nearly 95 percent of 316 infants with moderate to severe colic experienced improvements following two weeks of chiropractic care. More than half of these infants underwent prior treatment unsuccessfully (usually medication). Dr. Tracy Hudson, owner of Henlopen Chiropractic in Lewes, recently treated a newborn who was vomiting regularly and in gastrointestinal distress. She found a couple areas of subluxation (meaning vertebrae out of place) and performed an adjustment. Within 48 hours, the baby was sleeping through the night and eating normally.
  • Epilepsy: Chiropractic care may represent a nonpharmaceutical health care approach for pediatric epileptic patients, concluded a scientific literature review of 17 reports.
  • Immune System Function: At the end of six months, HIV patients who had received regular adjustments saw a 48 percent jump in their number of CD4+ T cells (white blood cells that protect the body from infection); the patients not adjusted experienced an 8 percent decrease in CD4+ T cells.
  • Infertility: A 30-year-old woman who dealt with infertility for eight years became pregnant following eight months of chiropractic care, according to a case study. Dr. Becky Leary, a chiropractor with Dover Family Chiropractic in Camden, had a recent similar success story with a patient having trouble conceiving coupled with back pain. “The nerves from the lower back go to the reproductive organs,” she explains.

Sometimes, though, the research proves to be less than conclusive for certain conditions. For example, one study on insomnia stated “convincing evidence has yet to be produced” on routine chiropractic care for patients with sleeping difficulties, and a literature review of 49 articles on spinal manipulative therapy and ear infections found no clear evidence to support its use in children—but no harm in trying.

The research also offers mixed results on asthma, with investigators in one study noting that some patients may benefit from chiropractic care as an adjunct, not a replacement, to traditional medical therapy.


EXPLAINING CHIROPRACTIC CARE

Chiropractic care focuses on disorders of the musculoskeletal and nervous systems and how they affect general health, according to the American Chiropractic Association. It’s used most frequently to treat neuromusculoskeletal complaints, such as pain in the back, neck and joints of the arms or legs, as well as headaches.

Spinal manipulation—aka chiropractic adjustment—is one of the most common therapeutic procedures. 

“The spine is the key to everything,” says Dr. Tracy Hudson, owner of Henlopen Chiropractic in Lewes. “If that’s not functioning properly, nothing will.”

The adjustment helps to restore joint mobility by manually applying a controlled force into joints that have become restricted in their movement from a tissue injury, which can be caused by a single traumatic event (think a car accident or poor shoveling technique) or through repetitive stresses (awkwardly hunched at a desk for hours on end). 

“In either case, injured tissues undergo physical and chemical changes that can cause inflammation, pain and diminished function for an individual,” describes the American Chiropractic Association, the largest professional association in the United States representing doctors of chiropractic. “Manipulation, or adjustment of the affected joint and tissues, restores mobility, thereby alleviating pain and muscle tightness, allowing tissues to heal.”
